Overview

Aledade partners with primary care physicians, providing support services and its proprietary, cloud-based data and workflow technology platform. This partnership allows practices to deliver high quality healthcare to patients in the best settings, participate in value-based care alternative payment models, and stay independent. Aledade operates Accountable Care Organizations across most of the United States in partnership with hundreds of primary care practices, including over 100 federally qualified health centers. Through this nationwide network, Aledade practices manage roughly USD $12 billion in healthcare spending through Medicare and other value-based contracts and provide care for over one million patients.

Our Perspective

In the U.S. healthcare system, increasing cost pressures and regulatory changes have accelerated the shift from fee-for-service to value-based care payment models. In particular, primary care is where we believe value-based care can have the greatest impact; primary care accounts for around six percent of total healthcare spending in the U.S., however, primary care physicians influence the majority of downstream costs. Aledade has developed a unique and scalable model for independent primary care physicians, as proven by its consistent ability to deliver shared savings while improving clinical quality and outcomes. The management team includes thought leaders in healthcare, technology, and regulatory policy. Aledade is led by CEO and co-founder Dr. Farzad Mostashari, the former National Coordinator for Health IT at the U.S. Department of Health and Human Services, and he also served as a distinguished expert at the Brookings Institute’s Engelberg Center for Health Care Reform. Farzad’s extensive career in health IT, health care policy and delivery, and more importantly, his insightful and thoughtful approach, position him well to partner with physicians to push forward the shift to value-based primary care.
